The coordinator helps eligible adults with disabilities transition into and maintain housing through person-centered planning, skill building, advocacy, and connections to community resources, supporting housing stability and reducing the risk of homelessness or institutionalization.

What you'll do

• Provide community-based housing transition and sustaining services
• meet with clients in their homes or community
• identify housing barriers and assist with applications and benefits
• help clients understand leases and prepare for move-in
• support housing retention, recertification, tenant skills, and household management
• maintain a caseload and required documentation
• provide transportation and crisis assistance as needed.
