What you'll do

• Create and update individualized plans of care
• administer care plans in collaboration with the Director of Nursing, clients, caregivers, physicians, and care team
• provide skilled nursing and private duty services
• conduct initial and reassessment nursing assessments
• orient and supervise aides
• perform quarterly supervision visits, skill checks, and annual evaluations
• document services and changes in client condition
• communicate with families and case managers
• travel locally and to other markets as needed.
