Hospice Registered Nurse (RN) Case Manager — Full-Time

What you'll do

• Provide hospice nursing care in patients’ homes and other care settings
• complete nursing assessments
• develop and update individualized care plans
• manage pain and symptoms
• monitor patient condition
• coordinate medications, treatments, supplies, and other needs
• educate patients and families
• coordinate with healthcare providers and interdisciplinary colleagues
• participate in IDG meetings
• maintain accurate clinical documentation.
