What you'll do

• Conduct initial and ongoing assessments
• provide weekly individual counseling
• facilitate individual, group, and family therapy as appropriate and within licensure scope
• develop treatment plans and case-management goals
• document client progress
• monitor outcomes and communicate with the treatment team
• develop discharge and aftercare plans
• coordinate with external partners
• maintain licenses and certifications
• transport clients as needed.
