MDS Coordinator

What you'll do

• Coordinate the facility’s Resident Assessment Instrument process under state and federal regulations
• complete MDS and supporting assessments and clinical documentation
• implement and evaluate resident care plans
• schedule and lead interdisciplinary care plan meetings
• audit records for supporting documentation
• educate staff as needed
• coordinate with health plans, physicians, and the interdisciplinary team on approvals and coverage criteria
• interpret regulations and coverage guidelines
• schedule assessment reference dates
• transmit assessments and resolve validation errors
• maintain assessment records and reports
• participate in resource utilization and triple-check meetings.

Why this role

Requires an active, unencumbered registered nurse license in the state and three years of RN experience, including supervisory experience. RAC training must be completed within six months; RAC certification is preferred.
