What you'll do
- Perform resident assessments and reassessments and monitor changes in condition
- communicate resident needs to the Health and Wellness Director
- when necessary, participate in hands-on care, medication administration, and nursing tasks
- schedule and assign caregiving and medication technician shifts
- supervise, train, and support care team members
- assist with physician orders
- maintain resident records, care plans, incident reports, and medical documentation
- coordinate care planning with onsite home health agencies
- address resident needs and complaints
- coordinate admissions and move-ins
- facilitate communication among residents, families, and healthcare providers.
