Hospice Chaplain

What you'll do

• Conduct a spiritual assessment within five days of hospice admission
• assist with development and implementation of the spiritual-care portion of the interdisciplinary care plan
• provide spiritual support according to patient and family wishes
• collaborate with local clergy when desired
• maintain clinical records, including evaluations, care plans, and notes
• evaluate responses to spiritual counseling
• coordinate visit scheduling with case managers weekly
• participate in IDG and QAPI meetings and activities
• conduct prayer, worship, and rituals as appropriate
• work with professionals on spiritual or ethical issues
• provide staff spiritual and bereavement support as needed
• educate the IDG on spiritual care
• provide funeral or memorial services when requested.
