What you'll do

• Maintain relationships with referring hospitals and outpatient providers
• coordinate intake with crisis residential programs and referral sources
• interview clients and recommend admission
• complete intake paperwork and obtain required documentation
• orient new clients to the facility and staff
• assess for involuntary hospitalization under WI 5150 after county certification and complete forms
• intervene in crises
• provide on-the-spot counseling
• conduct psychosocial groups and treatment and discharge planning
• support member safety and well-being.
