What you'll do

• Assess, plan, implement, coordinate, monitor, and evaluate care options and services

• provide discharge planning through referrals and multidisciplinary planning
• serve as a liaison between patients, families, and the healthcare team
• help ensure patient needs are met appropriately and promptly.
