Pharmacist — Home Infusion

Job overview

The Pharmacist works in person in Manchester, NH, from 8:00 a.m. to 6:00 p.m. Monday–Thursday, with an on-call rotation; compensation is $115,000–$145,000 per year. The role supports Vital Care’s home infusion therapy services as a member of the Pharmacy team. The pharmacist evaluates and monitors drug therapy for home-care patients, coordinates with healthcare professionals, and verifies medications and patient orders. The role supports safe, effective pharmaceutical care through patient assessment, consultation, documentation, and compliance with pharmacy standards and regulations.

What you'll do

• Evaluate and monitor drug therapy in collaboration with physicians, nurses, and other healthcare professionals
• follow pharmacy policies, laws, regulations, and accreditation standards
• participate in quality assurance
• coordinate referrals and admissions
• review, receive, and verify patient orders
• assist with home-care planning and therapeutic objectives
• assess patient, drug, and disease information
• monitor therapy outcomes using laboratory and clinical data
• consult with patients, families, and healthcare professionals
• document clinical activities and communications
• process and check medications, supplies, and equipment before dispensing
• maintain clinical knowledge through literature and professional learning.

Why this role
