Clinician (LMSW, LPCA, LMFTA) - Child/Adolescent PHP

Conducts comprehensive bio-psychosocial assessments and establishes care plans. Provides direct patient care including case management, care coordination, crisis intervention, counseling, and emotional support. Refers patients to appropriate levels of care and resources. Maintains timely patient-centered documentation.
