The Registered Nurse (RN) Case Manager, Home Health role is onsite across Lifespark’s Southwest Metro service area in Minnesota, including Waconia, Carver, Chaska, Jordan, Chanhassen, Savage, Minnetonka, and surrounding communities. The stated wage range is $88,000–$90,000 annually, depending on experience, plus a limited-time $2,500 signing bonus and mileage reimbursement of 72.5 cents per mile. Lifespark supports seniors in maintaining health, navigating care, and living more independently. The nurse assesses clients in their homes, develops and updates care plans, provides nursing care, and coordinates services with families, physicians, caregivers, and community partners. Case management and caregiver supervision help ensure that care responds to changing needs and follows applicable requirements.

What you'll do

• Assess clients and families to determine home-care needs
• reevaluate nursing needs and revise care plans as conditions change
• develop goals and preventive, therapeutic, and rehabilitative nursing actions with client and family input
• administer prescribed medications and treatments
• counsel and educate clients and families
• identify and implement discharge-planning needs
• coordinate assigned caseloads when designated as case manager
• communicate with physicians and community health contacts
• complete clinical notes and documentation
• supervise and evaluate home health aide care every two weeks
• manage supplies and appointments
• organize travel and document mileage.

Why this role

Schedule is Monday–Friday, 8:00 a.m.–5:00 p.m., with weekend and on-call rotation and two holidays per year in rotation. Requires a current Minnesota nursing license and graduation from an accredited nursing school; two years of RN experience is requested.
