The Bilingual Wraparound Specialist role is onsite in Houston, TX, at HOPE Clinic’s Aldine location. Pay is $18.00–$25.00 per hour. The role supports HOPE Clinic patients by addressing social barriers that affect health and by helping immigrants and refugees navigate the U.S. health system. The specialist assesses patients’ social needs, develops plans with them, and connects them with resources and follow-up support. This work helps patients address needs involving transportation, food, housing, education, and employment so they can access care and pursue improved health.

What you'll do

• Assess patients’ social needs using the PRAPARE tool
• develop plans with patients
• provide referrals, coaching, and follow-up
• maintain knowledge of social service resources
• respond to patient questions
• document patient information securely and accurately
• coordinate with clinics, specialists, physicians, management, and billing
• schedule patients
• attend community engagement activities and clinic events as needed
• protect confidential information and maintain HIPAA standards.
