What you'll do

• Complete required sections of the MDS
• create, document, and monitor care plans and lead care plan meetings
• ensure medical-record documentation supports MDS coding
• collaborate with the interdisciplinary team to assess resident needs and formulate care plans
• and ensure skilled certification and recertification documentation is accurate.
