Working with specialty clinicians, facilities, primary care providers, colleagues, and families as appropriate, the role supports coordinated care for older adults with neurocognitive, neurologic, or psychiatric conditions.

What you'll do

• Evaluate patients
• establish differential diagnoses, including Alzheimer’s, Parkinson’s, and vascular conditions
• conduct cognitive assessments
• provide condition-specific support therapy or insight-based therapy when indicated
• coordinate care plans with the facility, primary care provider, GuideStar colleagues, and family as indicated.
