Serenity Hospice is seeking a driven, high-impact Hospice Liaison to grow our presence in Tigard, OR. This is an onsite, field-based role located in Portland, OR. Compensation is not specified. Serenity Hospice is a healthcare company dedicated to providing compassionate end-of-life care. The role exists to develop strong referral partnerships, increase census, and position Serenity Hospice as the provider of choice. The Hospice Liaison contributes to the team by facilitating referrals, assisting with patient intake and admissions, and implementing marketing and education programs to increase awareness.

What you'll do

• Coordinates and facilitates referrals to Hospice care based on Hospice admission criteria
• Assist in the information gathering, education, and form completion aspects of patient intake, admissions, and transition to home
• Develop a consultative relationship, in assigned accounts by making effective customer contacts and presentations
• Develop and implement marketing and education programs to increase awareness of the company.
