What you'll do

• evaluate diagnoses, medications, behaviors, functional status, and care needs
• determine whether facilities can safely meet resident needs
• obtain and track insurance authorizations
• identify barriers before admission
• document acceptance or denial decisions
• communicate with hospitals, referral sources, and facility leadership
• match accepted residents with an appropriate facility and available bed
• monitor census, bed availability, and referral activity
• help move accepted referrals through admissions.
