What you'll do
- Conduct maternal and infant health assessments
- review medical histories and identify risks for intervention or referral
- monitor health during pregnancy and postpartum
- provide nursing consultation and participant education
- coordinate with medical providers and community partners
- assist with care plans and referrals
- participate in case reviews as requested
- maintain timely documentation and protect confidentiality under HIPAA and nursing standards.
