What you'll do
- Identify eligible individuals and maintain follow-up
- visit shelters, feeding sites, and sleeping sites with the outreach team

- connect participants with treatment, recovery housing, benefits, and community providers
- respond to urgent assistance requests, including at short notice and odd hours
- maintain case notes, client files, and HMIS records
- protect confidential information.
