The Registered Nurse Assessment Coordinator (RNAC) role is onsite in Johnstown, PA. Pay is $40–$42 per hour. The position supports Heritage Ridge Senior Living at Johnstown and its older adult residents. The RNAC determines residents’ appropriate level of care and coordinates assessment processes. The role supports timely, accurate assessments and documentation, participates in quality assurance, and helps ensure resident care plans are followed.

What you'll do

• Determine residents’ appropriate level of care
• schedule and oversee resident MDS assessments
• participate in pre-admission level-of-care decisions
• review MDS assessments and related forms for timely completion
• participate in the Quality Assessment and Assurance Program
• understand and follow resident care plans
• maintain accurate documentation, charting, reporting, and progress notes.
