What you'll do

• Review case referrals and patient information, including home visits, to determine hospice and home-care needs
• assign hospice personnel as needed
• consult the medical director about eligibility questions
• review cases through visits, conferences, records, and clinician services
• guide clinicians
• review diagnoses, prognosis, medications, procedures, and clinical course
• manage medical records and EMR workflow
• assist with care goals and plans
• attend case conferences
• conduct quarterly record reviews
• support hiring, orientation, and staff education
• participate in policy and quality-improvement work
• assess community health needs and support related programs and outreach.

Why this role

Requires current state registered nurse licensure and CPR certification. A BSN is preferred, or at least one year of professional nursing experience within the last three years; hospice or home-care experience and two years of management or supervisory experience are requested.
