What you'll do

• Provide weekly clinical support
• assist with assessments, treatment planning, and progress monitoring
• coordinate with therapists, psychiatric providers, PRP staff, case managers, and community providers
• track referrals
• support engagement, transitions, and continuity of care
• maintain timely electronic medical records
• ensure documentation meets agency, payer, and regulatory standards
• participate in supervision, audits, training, and quality improvement
• coach treatment team members and help resolve service barriers.
