Case Management Assistant

Job overview

The Case Management Assistant role is onsite in Sebastopol, CA. Pay is $23–$28 per hour. The role supports Sonoma Specialty Hospital’s Case Management department. The assistant provides administrative and coordination support that helps case managers manage utilization reviews, authorizations, referrals, and discharge arrangements. Accurate records and timely follow-up help coordinate patients’ transitions and post-acute services.

What you'll do

• Track concurrent review deadlines
• obtain clinical information from the EMR or managed-care forms and send it to payors
• confirm receipt and follow up for authorizations
• document continued-stay activities and communications in HMS
• assist with post-discharge referrals, services, transportation, discharge packets, and physician appointments
• answer phones, arrange meetings, and support department administration.
