Clinical Liaison | Private Duty

Job overview

The Clinical Liaison role is onsite in Knoxville, TN, with stated pay of $55,000–$65,000 per year. The role supports Adoration Private Duty and its home-based skilled private duty and home health services by coordinating patient transitions and referrals. The liaison helps maintain continuity of care by communicating patient needs and referral status among healthcare facilities, providers, and agency staff. The role also supports timely referral processing, provider education, and coordination before admission or resumption of care.

What you'll do

• Manage communication among physicians, social workers, discharge planners, hospital case managers, and agency staff
• relay patient care information and status updates
• track non-admits and communicate outcomes
• coordinate care needs and concerns before admission or resumption of care
• facilitate clinical orders and referrals
• build provider relationships
• conduct pre-discharge facility visits at physician request
• notify providers of discharge and upcoming admission
• manage assigned accounts
• educate providers on service capabilities
• document patient and payer information
• and support timely physician-order processing. An active nursing license is required
• LPN or RN is preferred. Reliable transportation, a valid driver's license, and current auto insurance are required.
