What you'll do

• Conduct initial evaluations, Start of Care visits, and monthly supervisory visits
• assess patients and update charts
• develop and revise goal-focused care plans
• provide home visits, instruction, and nursing treatments
• supervise aides
• communicate with physicians about progress and changes
• organize visit schedules
• share patient needs and care-plan updates with the supervising nurse
• share on-call duties in emergency or staffing situations.
