What you'll do

• Conduct comprehensive in-home assessments
• develop and manage individualized care plans with physicians and care teams
• provide skilled nursing care, medication management, treatments, and interventions
• coordinate care across therapists, aides, and physicians
• monitor progress and respond to changing conditions
• educate patients and families
• coordinate transitions and discharge planning
• maintain timely, accurate EMR documentation
• supervise and support home health aides as needed
• participate in on-call rotation as appropriate.
