What you'll do

• Coordinate multidisciplinary care for an assigned patient panel
• review records to diagnose and treat conditions
• provide care management and education to patients and families
• facilitate referrals
• manage acute and chronic conditions with multiple comorbidities
• follow healthcare guidelines and hospital standards
• collaborate with physicians, trainees, advanced practice professionals, and staff
• contribute to academic, educational, and research initiatives
• support community outreach.
