What you'll do

• Assess psychosocial risks, resources, and transitional-care needs
• collaborate with patients, families, physicians, and care teams on care progression and discharge plans
• identify and address delays and barriers
• manage discharge planning and post-acute referrals
• participate in interdisciplinary rounds and patient/family education
• document assessments and care coordination
• track avoidable days
• mentor or precept colleagues when needed.
