Student Health Services Nurse Case Manager

The Student Health Services Nurse Case Manager role is based onsite at Eagle Point School District 9 in White City, OR, and requires travel to schools within the district as needed. The nurse identifies and assesses student health needs and coordinates appropriate support for acute and chronic conditions. Working with school staff, families, and medical providers, the role develops health plans, connects students with resources, and helps reduce barriers to care, particularly for at-risk and special-needs students.

What you'll do

• Identify and assess student health needs
• provide health maintenance and disease-prevention instruction
• prepare classroom health plans with students’ medical providers and families
• refer students to appropriate resources
• collaborate with school departments, staff, and parents
• provide clinical direction as appropriate
• travel to schools within Eagle Point School District as needed.

Why this role

Position is for the 2026/27 school year only; full-time hours during the school year, with two weeks off at Christmas and July off. Current unencumbered Oregon RN license and current Basic Life Support card required.
