The role collaborates with nursing staff, residents, families, and interdisciplinary team members, and includes employee health and education responsibilities and participation in a Manager-on-Duty rotation.
What you'll do
- Coordinate care conferences and family meetings
- develop, implement, and update resident care plans
- complete and maintain MDS assessments and documentation
- monitor resident condition changes and communicate with nursing leadership
- support regulatory compliance, quality assurance, and clinical documentation
- assist with staffing coverage as needed
- participate in the Manager-on-Duty rotation
- track employee TB testing and health requirements
- conduct new-employee health assessments and contribute to staff education.
