What you'll do

• Research and arrange external services for residents
• schedule discharge meetings, home visits, home care, and therapy
• direct timely five-day discharge planning meetings for newly admitted residents
• assess support systems and home situations
• support bed management
• assist residents with social issues and care coordination
• and conduct customer satisfaction surveys.
