What you'll do

• Provide direct care to subacute rehabilitation residents, including assessment, planning, implementation, and evaluation of individualized care plans
• collaborate with interdisciplinary teams on care plans
• support high-quality patient outcomes
• participate in ongoing education and professional development
• contribute to a supportive work environment through communication and teamwork. Requires current Vermont RN licensure
• candidates with zero or more years of experience are considered.
