The Hospice Field Registered Nurse (RN) — Weekend is an onsite, full-time role based in Sulphur Springs, TX, providing care in patients’ homes or other care settings. Compensation is not specified. Choice Hospice provides hospice care for people with life-limiting illnesses and support for their families. The nurse assesses patient needs, develops and coordinates care plans, and works with an interdisciplinary team to support comfort, dignity, and continuity of care. The role also educates patients and families, guides hospice aides and licensed practical or vocational nurses, documents care, and performs assigned on-call responsibilities.

What you'll do

• Conduct initial and ongoing comprehensive assessments
• determine hospice admission and recertification eligibility
• develop, implement, and document care plans
• educate patients and families about disease processes, self-care, end-of-life care, nutrition, and dietary needs
• determine service scope and frequency
• provide clinical direction to hospice aides and LPNs/LVNs
• prepare clinical and progress notes
• communicate changes and outcomes to physicians and other personnel
• perform on-call responsibilities as assigned.
