What you'll do
- Provide nurse visits to home health patients.
- Perform initial comprehensive assessments and document findings.
- Obtain and document physician orders.
- Re-evaluate and revise plans of care in consultation with the Clinical Team Manager.
- Coordinate care with physicians and other team members.
- Perform re-certifications, transfers, and discharges.
- Supervise Home Health Aides.
