Hospice Clinical Nurse Liaison

What you'll do

• Build relationships with referral sources
• explain hospice services and eligibility
• provide education to patients and families
• conduct educational in-services
• collaborate with medical staff and referral sources on eligibility
• coordinate transitions to inpatient, hospital, home, or facility-based hospice care
• obtain consent, support discharge planning, coordinate transportation, document referrals and follow-ups, and help initiate admissions and supply orders.
