What you'll do

• Accept and treat patients in the designated geographic area
• provide one-to-one treatment
• evaluate patients and develop individualized plans based on physician orders and patient needs
• deliver care according to professional standards and the prescribed plan
• educate family members and caregivers
• conduct and document initial examinations
• document visits promptly
• evaluate treatment effects and adjust care
• treat geriatric patients
• travel to patients' homes within the assigned territory.
