What you'll do

• Develop and implement individualized service plans with clients, caregivers, and interdisciplinary teams
• facilitate skill-building activities and structured programs
• monitor and document progress
• follow licensing, organizational, health, and safety requirements
• provide support with mobility, self-care, or behavior as specified in individual plans
• maintain confidential records
• collaborate with case managers, therapists, job coaches, and support staff
• participate in meetings and professional development
• support site events and community outings.
