Hospice Liaison

What you'll do

• Develop relationships with physicians, discharge planners, case managers, and social workers
• educate providers and community partners about hospice and palliative care
• coordinate with internal teams on admissions and transitions
• conduct outreach presentations, in-services, and networking
• represent the organization at community events and meetings
• maintain outreach, referral, and communication records.
