What you'll do
- Manage an assigned home-care caseload
- complete start-of-care and recurring assessments and supervisory visits
- develop and update plans of care
- orient, train, mentor, and support caregivers
- monitor compliance and documentation
- coordinate staffing and coverage plans
- communicate with clients, families, providers, and referral partners
- provide direct client care and field support as needed
- participate in the caregiver support on-call rotation.
