What you'll do

• Act as a liaison to support ongoing care management and problem-solving
• review physician-ordered plans of care and OASIS assessments for completeness and accuracy
• review resumption-of-care and discharge assessment data and approved orders as needed
• complete assigned HCHB tasks and coordination-note workflow within set timeframes
• enter new patient orders and update plans of care and calendars as appropriate
• review and approve new interim orders before they are sent to physicians.
