The role supports the Acute Rehabilitation team and patients recovering from serious illness, injury, surgery, and other complex medical events. The nurse provides and coordinates rehabilitation-focused care, monitors clinical status, and works with an interdisciplinary team to help patients progress toward greater independence and a safe transition to the next level of care. Patient and family education, advocacy, documentation, and support for nursing colleagues are also part of the role.

What you'll do

• Provide direct acute rehabilitation nursing care
• assess and reassess patients
• develop, implement, and evaluate individualized care plans
• collaborate with physicians, therapists, case management, and the rehabilitation team
• monitor and report clinical changes
• administer medications and treatments
• support mobility and self-care goals
• educate patients and families
• document in the electronic medical record
• delegate appropriate responsibilities
• guide and mentor nursing team members
• participate in quality improvement.
