What you'll do

• Guide treatment plans with the interdisciplinary team based on patient needs and preferences
• coordinate tentative discharge and contingency plans
• participate in discharge planning
• monitor patient experience and service appropriateness
• facilitate weekly team conferences and treatment plan modifications
• complete required documentation
• review case management reports
• educate patients and families
• arrange resources and referrals
• participate in utilization review and the case management on-call schedule as needed.

Why this role

Must be qualified to independently complete assessments within the scope of the relevant discipline and hold an active license when required. CCM or ACM certification is required or must be obtained within two years for the Case Manager II position; degree requirements vary by discipline.
