What you'll do

• Assess patients’ long-term care needs and social support challenges
• advise on living arrangements, safety concerns, and available services
• onboard new patients
• maintain knowledge of senior-care resources
• coordinate access among patients, families, and community resources
• provide outreach and education
• support care transitions and access to care management, advance care planning, and hospice services
• document activities
• participate in community events
• and support referral development and market expansion.
