What you'll do

• Assess psychosocial status and communicate findings to the interdisciplinary group
• evaluate family dynamics and caregiver needs
• counsel patients and families and provide crisis intervention when necessary
• educate the team about cultural needs
• maintain clinical records
• educate and assist with advance directives
• provide information and referrals
• coordinate with community agencies
• participate in individualized care planning and interdisciplinary meetings
• support quality improvement activities
• assist with funeral, financial, legal, and healthcare decision planning.

Why this role
