What you'll do

• Provide skilled nursing assessment and intervention
• update and review care plans
• respond to urgent patient and family needs with triage staff
• initiate crisis care as needed
• report significant changes
• educate patients and families
• administer and titrate medications under orders
• manage patient and residence contingency medications
• record orders and communications
• delegate nursing duties to LPNs and supervise CNA care
• complete admissions as needed
• conduct telephone triage and follow-up
• coordinate volunteer deliveries.

Why this role

Full-time 0.8 FTE, 32 hours weekly: two 12-hour shifts from 7 p.m.–7:30 a.m. and one 8-hour shift from 11 p.m.–7 a.m. Includes every other weekend, two holidays per year, and an on-call rotation of approximately 10–15 shifts annually.
