Nursing Coordinator — Home Infusion

What you'll do

• Schedule nurses for new and ongoing patient infusion visits
• coordinate care with nurses and pharmacy
• transfer referral data
• maintain accurate scheduling and patient records
• coordinate and confirm appointments
• provide updated prescription orders to nurses when applicable
• document supply requests
• collect and review nursing reports and invoices
• support nurse and agency credentialing
• maintain compliance documentation
• escalate clinical concerns and nursing issues.
