Case Manager – Recovery Services

The case manager supports adults experiencing substance use disorders by coordinating services that promote recovery and resiliency. The role assesses client needs, provides advocacy and crisis support, and connects clients with community resources, family, employers, and other providers.

What you'll do

• Assess, plan, implement, coordinate, monitor, and evaluate services to meet client needs
• provide symptom monitoring, crisis management, and advocacy
• facilitate education and sober-living skills
• provide client and family education
• document clinical progress
• connect clients with referral services, employers, family members, providers, and other resources
• collaborate with agency and community partners.

Why this role

High school diploma or GED is the minimum education requirement; a bachelor’s degree in social work, psychology, or a related human services field is highly preferred. Ohio CDCA credential and a valid driver’s license, vehicle insurance, and compliant driving record are required.
