What you'll do

• Educate referral sources on hospice benefits, clinical outcomes, and eligibility criteria.
• Meet with patients and families to explain hospice services and coordinate the admission process.
• Gather and submit clinical information to support patient terminal prognosis for RN review.
• Collaborate with branch staff, field sales teams, and referral sources to ensure timely care coordination.
• Participate in community outreach through speaking engagements, in-services, and healthcare events.
• Support compliance with federal, state, and Medicare hospice regulations.
• Document activities accurately in the contact management system (CRM).
