What you'll do
- Coordinate MDS assessments and care plans
- notify care team members of due dates and ensure their portions are completed
- develop or revise care plans quarterly and after significant changes
- coordinate care plan meetings
- submit accurate, timely MDS assessments under Medicare, Medicaid, and applicable state and federal guidelines
- protect resident privacy and rights
- communicate changes in resident condition
- monitor care during the MDS process and report concerns
- and assist with pre-admission screening as needed.
