Community Home Care Coordinator

What you'll do

• Check in with clients and coordinate care-plan and schedule updates
• conduct home visits and nonclinical assessments
• explain services and rates and complete enrollment paperwork
• support caregivers through check-ins, observation, and coaching
• introduce caregivers to clients
• complete skills assessments and case-specific training
• help fill open shifts
• build relationships with residents, families, and community staff
• maintain AxisCare documentation and attend weekly meetings.
